m900 rear disc

I know the rear brakes on the old monsters aren't renowned for being great; but mine is particularly naff. Very little feel, minimal stopping and it chews through pads like there's no tomorrow.
Looking at the disc - it feels like an old vinyl lp! Extremely rough (probably why it eats brake pads).

Am looking to replace the disc and pads - any recommendations?
