How hard or how much would it cost to have something like this done? (the S4)

One issue is tyre clearance, there ain't much!!

How do the DD guys get the underseat set up?

One of the belgium MOB has underseat cans with a dual side swinger. Can be done but I would imagine you have to have flat cans or narrow diameter with a spread apart. Also the hardest part is the verticals and no pillion and rigid as possible rear shock.
That's Yves' S4, made himself from some 1098 end cans
